Effect of margin width on local recurrence in women with triple-negative breast cancer

Rates of local recurrence following breast conserving therapy differ between breast cancer subtypes, with the highest rates seen in triple-negative cancers and therefore it has been postulated that wider negative margins may improve local control within this subtype.  We compared 535 triple-negative cancers with negative margins (no ink on tumor) of <2mm and >2mm and found no significant difference in rates of local recurrence at 5 years.  This data, taken with reports that show similar rates of local recurrence in triple-negative cancers treated with mastectomy (the largest possible margin) or lumpectomy, suggest that bad biology is unlikely to be overcome with more extensive surgery.
